Breast Implant Size Equals Bra Cup Size
Every day that I meet with women for breast augmentation consultations I am asked about which implant size will make them a B, C, or D-cup. Unfortunately there is no magic formula for going from one bra cup size to another. There have been many ways described including filling bags with water, placing them in a bra and looking in the mirror, using rice instead of water, placing actual implants in a bra while in the office…etc. I have known none to be very accurate as anything placed in a bra, sitting on top of the already present breast, gives false projection and can lead to choosing a size that may be to small once placed in the body. I have found the best way to determine implant size is to first listen to the patient and then look at pictures with them. These pictures could be from our before and after album, magazines, or the internet, wherever the patient finds the best representation of what they are trying to achieve. Looking at pictures removes the fear of a letter of the alphabet and gives a more insightful look into what someone is really thinking. It opens the opportunity to discuss size, shape and look of the breast.
Pregnancy and Breast Implants
Periodically I will have a patient call that has already undergone breast augmentation who becomes pregnant. They are typically apprehensive about what will happen to their breasts due to weight gain and loss, hormone changes and when and if they breast feed. I make every effort to discuss these concerns prior to their original surgery but we are always here to answer questions that may come arise.
Breast implants have not been shown to be a risk to the developing fetus. It is also safe to breastfeed should you decide this is your desire. This is true for both saline and silicone implants.
As for what will happen to the breasts after pregnancy, it is impossible to predict. Some women will have their skin return to their pre-pregnancy form and have little change in the look of the breasts. Others unfortunately may experience stretching of the skin, and/or increase in breast size. These women may desire revision of their original surgery or additional procedures such as a breast lift (mastopexy). This determination is best decided at least 3-6 months after pregnancy or breast feeding has finished.

Which Breast Augmentation Approach Makes the Most Sense
Over the past 15 years, I have done over a thousand breast augmentation consults and almost as many surgeries. One of the most common questions I am asked is which is the best incision to use. Every surgeon has his preference as do I. There are three commonly used incisions for the placement of breast implants and one much less common approach. The common three are inframammary (under the breast), axillary (under the arm), and infra-areolar (at the base of the areola). The uncommon is when the implant is placed via the umbilicus (belly-button). Of these I have found the incision at the base of the areola to be the most advantageous as it is central to the breast mound, the most consistent healing, and never seen in any type of clothing. I find this approach the best way for me to create a closer, more natural appearing cleavage as well. Concerns about this incision are most commonly related to the potential for loss of sensation. All the incisions have an equal incidence of this complication as the real reason for loss of sensation is related to the stretching of the nerves, not where the skin is cut. To put you at ease the risk for complete and permanent loss of nipple sensation is quoted to be 1% or less.
